Stroke Pathway Package

Developed in partnership with the Grey-Bruce District Stroke Centre, the main goals of this pathway are to improve:

• Use of the best practice guidelines for stroke
• Length of stay to MOHLTC guidelines
• Consistent patient education
• Communication across sites for transferred patients and into CCAC/LTC
